FRESHMEN (NEW ENROLLEES)

• Form 138 (School Card) or Certificate of Rating (for ALS passers);
• Certificate of Good Moral Charater;
• Original PSA-Authenticated Certificate of Live Birth;
• Original PSA-Authenticated Marriage Certificate (For married women);
• Original Form 1A and/or Original Form 3A from the local Civil Registrar (if the PSA-Authenticated documents are illegible);
• Must pass the qualifying examination; and
• Must pass the interview by the Guidance Counselor.

CONTINUING STUDENTS

• Graduate from the preceding semester; and
• Certification from the Registrar (for students who will enroll Practicum)

SHIFTEES AND RETURNEES

• Notice of admission by the College/Department; and
• Evaluation of subjects, grades and curriculum.

TRANSFEREES

• Certificate of Transfer Credential / Honorable Dismissal;
• Transcript of Records (TOR);
• Certificate of Good Moral Charater;
• Original PSA-Authenticated Certificate of Live Birth;
• Original PSA-Authenticated Marriage Certificate (For married women);
• Original Form 1A and/or Original Form 3A from the local Civil Registrar (if the PSA-Authenticated documents are illegible);
• Must pass the qualifying examination; and
• Must pass the interview by the Guidance Counselor.

CROSS-ENROLEES

• Permit to cross-enroll;
• Certificate of Good Moral Character; and
• Notice of admission by the College/Department
